New to Mize?

Registration and Enrollment will take place July 25 and 26.

• July 25 from 12:00 p.m. to 6:30 p.m.
• July 26 from 8:00 a.m. to 11:30 a.m.
   

If your child will be new to Mize Elementary in grades K-5 for the 2024-25 school year, please print, complete, and return the appropriate enrollment forms to Mize as soon as possible. 

In addition to these forms, we will also need a (Xerox) copy of your child's birth certificate and a copy of your proof of residency (e.g. utility bill or front page of build/sales contract).  Your child's doctor can provide a copy of immunization records in place of the Kansas Certificate of Immunizations and a copy of your child's latest physical in place of the Health Assessment for Children and Youth form. (Please note, we only need physicals for children who are under the age of 10 and moving from out of state.)

Once we receive all of your registration documents, we will enter your student(s) into our computer system, and then we can send you your Skyward Family Access login information.  Skyward is where you can pay district/school fees, add money to lunch accounts, view your student's grades, attendance, and more!  You are welcome to scan/email your enrollment documents to jminihan@usd232.org fax them to 913-441-0880, drop them off in the front office, or mail them to Mize Elementary, Attn: Janet Minihan, 7301 Mize Rd., Shawnee, KS  66227.
